The problem
Decisions no one can reconstruct
An autonomous system takes an action. Six weeks later nobody can produce the trail: what it observed, what it was permitted to do, who authorised it, and whether the result was ever independently checked.
Governance that is only documentation
Policies, risk registers and model cards describe intent. They do not demonstrate that a control actually operated on a specific decision on a specific day. Reviewers increasingly ask for the latter.
Green dashboards, unknown state
Most operational dashboards show liveness, not truth. A pipeline can report healthy while the system it feeds has stopped producing verifiable outcomes entirely.

How assurance works here
- Producer ≠ verifier
- The party that produces a result is never the party that certifies it. This applies to our own work as strictly as to yours.
- Evidence before claims
- A result is stated only at the scope it was measured. Where a value cannot be established, it is recorded as unknown — never smoothed into a passing state.
- Append-only records
- Corrections are new records, not edits. A sealed artifact is never rewritten, so the history of what was believed and when remains intact.
- Independent re-derivation
- Material findings are re-derived by a second, structurally different method before they are relied upon.
- Isolated by design
- This website is static content with no database and no application code. No public route reaches any internal system.
